How To Refer A Patient
For your convenience, we have several options for referring patients. Please call 951-652-2252 to schedule an appointment, or fax your completed referral form to 951-925-9252. You may download our GI Excellence Physician Referral Form with the button below.
The Following Information Is Needed To Make An Appointment For Your Patient:
- Patient Information: Patient name, address, birth date, and phone number.
- Referring Physician Information: Physician name and clinic, with clinic address and phone number.
- Reason for the Referral: Has the patient been diagnosed? What would you like the consultant to answer?
- Preferred Method of Communication: Phone, fax, or email.
Physician Referrals & Hospital Consultations
We care about our relationship with you, your patients, and your office staff. That’s why we work hard to keep you informed of your patients’ care by providing detailed reports from diagnosis to treatment and follow up.
Prior To The Appointment
Prior to the appointment, the following information must be submitted to the appointment scheduler:
- Image Studies, including CT, MRI, or Ultrasound scans within the last year.
- Medical records pertinent to diagnosis, including notes from previous surgeries and hospital discharge summaries.
- The completed GI Excellence Physician Referral form.
